Calcium is an essential mineral that plays a crucial role in various physiological functions, including bone health, nerve transmission, muscle contraction, blood clotting, and enzyme regulation. But when the levels of this mineral drop to abnormally low, you suffer from hypocalcemia. To know more about this condition, we spoke to Dr Ravi Shankerji Kesari, MD – Physician, General Physician, Apollo Spectra Hospital, Bengaluru.
Causes of Hypocalcemia
“Hypocalcemia is a medical condition characterised by an abnormally low level of calcium in the blood,” said Dr Kesari. Here’re the prevalent causes of hypocalcemia, as per Dr Kesai.
Hypoparathyroidism
“This occurs when the parathyroid glands, located near the thyroid gland in the neck, produce insufficient amounts of Parathyroid Hormone (PTH), which regulates calcium and phosphorus levels,” said Dr Kesai.
Chronic Kidney Disease
“The kidneys play a key role in regulating calcium levels in the blood. In cases of impaired kidney function, calcium homeostasis may be disrupted, leading to hypocalcemia,” said the doctor.

Medications
Dr Kesari said stated that certain medications, such as loop diuretics, bisphosphonates, and antiepileptic drugs, can interfere with calcium metabolism and lead to hypocalcemia.
Acute Pancreatitis
Dr Kesari said that acute pancreatitis can also lead to hypocalcemia. “This condition can lead to the deposition of calcium in the pancreas, reducing blood calcium levels,” he highlighted.
Malabsorption Syndromes
“Conditions like celiac disease, Crohn’s disease, or surgical removal of the intestines can impair calcium absorption,” he said.
Hypomagnesemia
“Low levels of magnesium can hinder the body’s ability to maintain proper calcium levels, leading to hypocalcemia,” said the doctor.
Vitamin D Deficiency
Dr Kesari said that vitamin D is necessary for the absorption of calcium from the intestines. “A lack of sufficient sunlight exposure or inadequate dietary intake of vitamin D can lead to hypocalcemia,” he mentioned.
Alcoholism
Alcohol is infamous for its many ill effects on health. Dr Kesari said that its excessive consumption can affect calcium metabolism and contribute to hypocalcemia.

Treatment and Management of Hypocalcemia
Here’s how you can manage hypocalcemia, as per Dr Kesari.
- Calcium and Vitamin D Supplements: Oral calcium supplements, often combined with vitamin D, are commonly prescribed to raise blood calcium levels.
- Treat Underlying Causes: Treating the root cause of hypocalcemia, such as addressing kidney dysfunction or vitamin deficiencies, is crucial for effective management.
- Parathyroid Hormone Replacement: In cases of hypoparathyroidism, synthetic PTH may be administered to help regulate calcium levels.
- Magnesium Replacement: Correcting low magnesium levels can indirectly improve calcium levels.
- Dietary Modifications: Increasing the intake of calcium-rich foods like dairy products, leafy greens, and fortified foods can help improve calcium levels.
- Medication Adjustment: If medication side effects are contributing to hypocalcemia, a healthcare provider might adjust or switch medications.
- Regular Monitoring: You should go for regular blood tests to monitor calcium levels and adjust treatment as needed.
Conclusion
Dr Kesari emphasised that the treatment and management should be personalised based on the underlying cause and the individual’s overall health. If you suspect you have hypocalcemia or are experiencing symptoms, such as muscle cramps, numbness or tingling, seizures, or abnormal heart rhythms, seek medical attention for proper diagnosis and management.
